POLST and DNR requirements
Unlike other forms that can be self-created and legally binding by self-signing or by using witnesses or a notary public (an advance directive or POA, for example), POLST and DNR forms require the review and signature of a healthcare provider. Again, these forms require a healthcare provider's signature. The forms are invalid if not signed by a provider.
Therefore, although these forms may be available for viewing, downloading, printing, and completing, it is strongly recommended that your loved one get these forms directly from the provider who will be reviewing them with your loved one and signing them.
A DNR order or POLST, once completed and signed, can only be revoked by:
your loved one
your loved one's primary care provider
A DNR order or POLST cannot be revoked by:
any other healthcare providers
any family member
any power of attorney
any other healthcare representative
Forms, languages
Some states simply use the National POLST form and not a POLST form specific to that state. The National POLST form is acceptable for states with POLST programs but without specific forms.
**While POLST and DNR forms are available in various languages for ease of education and understanding, the signed forms must be in English so healthcare personnel can read them and follow the orders without the need for a translator.**
